As SLCC monitors the spread of COVID-19, we are requiring members of the SLCC community to fill out this form if they have tested positive for COVID-19, tested negative for COVID-19 or are awaiting the results of a COVID-19 test. This information will allow the College to reduce the risk of any potential spread of COVID-19 and promote self-quarantine/self-isolation and social distancing best practices. The College will not release personal information about students, faculty, and staff utilizing this form in accordance with HIPAA and FERPA.
Currently, anyone experiencing flu-like symptoms (which now includes fever, cough, shortness of breath, sore throat, muscle aches and chills, or decreased sense of smell or taste) should be tested for COVID-19.
If you are experiencing symptoms and awaiting testing:
If you have been tested for COVID, have symptoms or possible exposure please complete this self report form.
You do not need to complete this form under the following circumstances:
- You tested due to travel requirements, pre-surgery or other reasons not related to symptoms or exposure, and the results are negative.
- You have not been in close contact for more than 15 minutes with someone who has tested positive and you have no symptoms.
