As SLCC monitors the spread of COVID-19, we are requiring members of the SLCC community to fill out this form if they have tested positive for COVID-19, have symptoms of COVID-19, or have experienced direct exposure to someone who has tested positive for the virus. This information will allow the college to reduce the spread of COVID-19 and promote quarantine/isolation and social distancing. SLCC will not release personal information about the students, faculty, and staff utilizing this form in accordance with HIPAA and FERPA.
Anyone experiencing flu-like symptoms (fever, cough, shortness of breath, sore throat, muscle aches, chills, or a decreased sense of smell or taste) should be tested for COVID-19.

You do not need to complete this form if you tested negative for COVID-19 OR you have not been in close contact for more than 15 minutes with someone who has tested positive and you have no symptoms.
